General Information
All hearings are held at the College of Nurses of Ontario (Hearing Room), 101 Davenport Rd., Toronto, Ontario.
This schedule is subject to change sometimes on a daily basis. Please contact the Communications Department at the College to confirm the hearing schedule.
Important Notice
Visitors and media* are prohibited from carrying into or using in the Discipline Hearing Room any audio/visual equipment, including video cameras, still photography equipment, laptop computers, and tape recorders.
* Media who are reporting on a hearing may use a tape recorder for the purpose of note taking only, and not for broadcast by any means.
Reporters are to advise the Hearing Clerk or Communications Department if they will be using a tape recorder
The Hearing Panel will limit the use of tape recorders, if necessary, to avoid disruption of the proceedings

Nature of Allegations
- Found guilty of offences that are relevant to her suitability to practice nursing, namely:
- that on or about the 28th day of August in the year 2005 in the City of Ottawa, did by deceit, falsehood or other fraudulent means, attempt to defraud a drug store of medicine of a value not exceeding five thousand dollars, contrary to Section 463, clause (d) of the Criminal Code of Canada; and
- that on or about the 9th day of November in the year 2005 at the City of Ottawa, did by deceit, falsehood or other fraudulent means, attempt to defraud a drug store of medicine of a value not exceeding five thousand dollars, contrary to Section 463, clause (d) of the Criminal Code of Canada.
- Contravened a standard of practice of the profession or failed to meet the standards of practice of the profession with respect to the administration of medications and/or the documentation of the administration of medications, in that she:
- signed an Individual Narcotic Medication Record indicating that she administered Tylenol 2 to a client at 9:00 p.m., when she was not at the facility at that time; and/or
- signed a Medication Administration Record indicating that she administered Tylenol 3 to a client at 9:00 p.m., when she was not at the facility at that time; and/or
- signed a Medication Administration Record indicating that she administered Clonazepam to a client at 9:00 p.m., when she was not at the facility at that time.
- Failed to keep records as required, in that she:
- signed an Individual Narcotic Medication Record indicating that she administered Tylenol 2 to a client at 9:00 p.m., when she was not at the facility at that time; and/or
- signed a Medication Administration Record indicating that she administered Tylenol 3 to a client at 9:00 p.m., when she was not at the facility at that time; and/or
- signed a Medication Administration Record indicating that she administered Clonazepam to a client at 9:00 p.m., when she was not at the facility at that time.
- Falsified a record relating to her practice, in that she:
- signed an Individual Narcotic Medication Record indicating that she administered Tylenol 2 to a client on or about at 9:00 p.m., when she was not at the facility at that time; and/or
- signed a Medication Administration Record indicating that she administered Tylenol 3 to a client at 9:00 p.m., when she was not at the facility at that time; and/or
- signed a Medication Administration Record indicating that she administered Clonazepam to a client at 9:00 p.m., when she was not at the facility at that time.
- Signed or issued, in her professional capacity, a document that she knew or ought to have known contained a false or misleading statement, in that she:
- signed an Individual Narcotic Medication Record indicating that she administered Tylenol 2 to a client at 9:00 p.m., when she was not at the facility at that time; and/or
- signed a Medication Administration Record indicating that she administered Tylenol 3 to a client at 9:00 p.m., when she was not at the facility at that time; and/or
- signed a Medication Administration Record indicating that she administered Clonazepam to a client at 9:00 p.m., when she was not at the facility at that time.
- Misappropriated property from a client or workplace, namely she misappropriated:
- a prescription pad from her workplace on or around August 2005; and/or
- a dose of Clonozapam from a client on or about November 9, 2005; and/or
- a dose of Tylenol and a dose of Zyprexa from a client on or about November 9, 2005.
- Engaged in conduct or performed acts, relevant to the practice of nursing that, having regard to all the circumstances, would reasonably be regarded by members as disgraceful, dishonourable or unprofessional in that:
- on or about August 28, 2005 in the City of Ottawa, she attempted to defraud a drug store of medicine by using a forged prescription; and/or
- on or about November 9, 2005 in the City of Ottawa, she attempted to defraud a drug store of medicine by using a forged prescription; and/or
- on or about November 9, 2005, she signed an Individual Narcotic Medication Record indicating that she administered Tylenol 2 to a client at 9:00 p.m., when you was not at the facility at that time; and/or
- on or about November 9, 2005, she signed a Medication Administration Record indicating that she administered Tylenol 3 to a client at 9:00 p.m., when she was not at the facility at that time; and/or
- on or about November 9, 2005, she signed a Medication Administration Record indicating that she administered Clonazepam to a client at 9:00 p.m., when she was not at the facility at that time; and/or
- on or around August 2005, she misappropriated a prescription pad from her workplace; and/or
- on or about November 9, 2005, she misappropriated property from a client or workplace, namely a dose of Clonozapam belonging to a client; and/or
- on or about the November 9, 2005, she misappropriated property from a client or workplace, namely a dose of Tylenol and Zyprexa belonging to a client.

Avis important
Il est interdit aux visiteurs et aux médias* d’apporter ou d’utiliser dans la salle d’audiences tout équipement audiovisuel, y compris caméras vidéo, appareils photo, ordinateurs portables et magnétophones.
* Il est permis aux médias qui préparent un reportage sur une audience d’utiliser un magnétophone pour la prise de notes, mais non aux fins de diffusion publique.
Les journalistes qui souhaitent enregistrer l’audience doivent en informer le greffier de l’audience ou le Service des communications.
Le jury peut restreindre l’usage de magnétophones s’il juge que cela perturbera le déroulement de l’audience.
