New parking permit form

NPs who certify parking permits for clients need to ensure they sign the right form.

In January 2006, the Ministry of Transportation introduced the Accessible Parking Permit (APP) program to replace the Disabled Person Parking Permit (DPPP) program. The APP program uses different eligibility criteria to assess applicants.

NPs and other recognized health practitioners must use the new “Application for Accessible Parking Permit” form (SRLV-129 07-08). This form is available on the Ministry of Transportation website, http://www.mto.gov.on.ca/english/dandv/vehicle/app.pdf, and at any ServiceOntario driver and vehicle licence issuing office.

All application forms certified after Jan. 2, 2008 must be on the new form. As of Feb. 1, 2008, ServiceOntario will only accept the new application
form. ServiceOntario will accept the previous APP form (SRLV-129 05-11) if it was certified by a recognized health practitioner prior to Jan. 2, 2008, and received by Jan. 31, 2008.
