You have indicated that you have registered in the jurisdiction where you completed your nursing program.

Please complete Section 1 of the Verification of Original Registration form (B-1). Send the form to the registration/licensing body with which you were/are registered.

It should then be returned directly by this registration/licensing body to the College of Nurses of Ontario.
