Declaration of Registration Requirements

You must complete a Declaration of Registration Requirements (Form A-5 in your Application Package), and answer the questions below.

Note: Your registration may be denied if you fail to report relevant information about these requirements. You must also report immediately any changes (e.g., criminal convictions, disciplinary action, physical or mental conditions/illness) that occur after the date of your application.
