Submission of Documents to Determine Suitability to Practise
You have indicated one or more of the following:
- Denied Registration or Licensure – You have indicated that your registration/licensure has been denied by an authority in a province, territory, state or country.
- Registration Revoked, Surrendered or Restricted – You have indicated that your registration has been revoked, surrendered, restricted, subjected to individual terms and conditions by a registration or licensing authority or another health profession in Ontario, or in another province, territory or country.
- Currently Under Investigation – You have indicated that you are under investigation, or involved in proceedings that could result in the encumbrance of your registration or license, by a registration or licensing authority for nursing or another health profession in Ontario, or another province, territory, state or country.
- Convicted of a Criminal Offence – You have indicated that you have been convicted of a criminal offence or an offence under the Narcotic Control Act (Canada) or the Food and Drug Act (Canada).
- Suffer from a Physical or Mental Illness – You have indicated that you suffer from a physical or mental condition or illness that may affect your ability to practise nursing.
