Change Request Forms: Proposed Drug, Laboratory Test and Diagnostic Test Lists
The College regularly proposes changes to the drug, laboratory test and diagnostic test lists to allow RN(EC)s to provide appropriate and timely care to clients without unnecessary referrals or duplication of services.
RN(EC)s can participate in future revisions of the schedules by downloading and completing a Change Request Form.

Please support your recommended changes with evidence-based rationale, including research articles.
Submit the forms along with accompanying research articles to the College’s address or fax number, which are listed on the forms.
