How do I replace a lost Annual Payment Card (APC)?
To request another APC, call Customer Service at 416 928-0900 (toll-free in Ontario at 1 800 387-5526) and have the following information ready:
- Ontario Registration Number
- First and last name
- Maiden or former name
- Date of birth
- Name of school of nursing
- Year of graduation
- Mailing Address and telephone number
A replacement APC costs $5.25 (GST incl.) and can be paid for with Visa, Mastercard, American Express, cheque or money order.
