I'm interested in practising nursing in Ontario. How do I request an assessment package?

The College's assessment package helps you determine your eligibility to practise nursing in Ontario. To receive this package, provide the following information to the College:

  1. First and last name
  2. Maiden or former name
  3. Date of birth
  4. Gender
  5. Mailing address and telephone number
  6. The year you graduated from your nursing program
  7. Country or province where you completed your nursing program
  8. Type of education received (i.e., Degree or Diploma)
  9. Type of registration you are applying for: Registered Nurse (RN) or Registered Practical Nurse (RPN)

Send this information to the College at:

We will then send you an Assessment Package that includes:

  1. Forms for payment, school information and employment information.
  2. A form to be sent to your employer(s) to verify your work status.
  3. A form to be sent to the nursing school where you took your original program, to verify your education.
  4. A form to be sent to the nursing board where you were originally registered, to verify your registration.
  5. A form to be sent to a nursing board(s) that you have been registered other than your original, if applicable.
