What information do I need to provide when calling the College for details from my file?

CNO requires you to provide the following information:

  1. Ontario registration or application number
  2. First and last name
  3. Date of birth
  4. School of nursing
  5. Year of graduation
  6. Mailing address and telephone number

Your request can not be completed without all of this information. In addition, e-mails not containing this information will be returned with a request that you provide us with these details. Please include your original e-mail request with your reply.
